1- I checked the opt date and time for the last opt-out on the profile and noticed that this was a manual opt-out through a CSV upload.

2- I checked the Audit logs for opt-out actions done around that date and time and found it.

Details below.

I checked the logs and I see that there was a bulk opt-out action taken by Elizabeth Burns on 7/22/2022 at 2:25 PM ET to unsubscribe a CSV list of phone numbers. So, probably this phone number was mistakenly added to the opt-out list.

The name of the CSV file used was "20220719_ea_phone_status_opt-out_group_20220722_140957.csv".
